Quality Home Health Care in Lebanon County

What we help with

We support older adults, people recovering after a hospital stay, and adults with special needs who want to remain safe and comfortable at home - with help that can scale up or down as needs change.

Services at a glance

  • Personal care: bathing, grooming, dressing, toileting, and mobility support

  • Daily living support: meal prep, light housekeeping, laundry, and errands/grocery shopping

  • Companionship and supervision to support safety and routine

  • Respite care to give family caregivers planned relief

  • Specialty support based on assessed needs and care plan

  • Transition support after a hospital stay or when returning home from a facility

What to expect

  1. Free information orientation (a quick, helpful call) - We’ll ask a few questions to understand your situation and explain options for personal care, specialty care, respite, and companionship.

    • We’ll review assistance options, including participation in multiple government programs and a budget-friendly private pay package.

    • Helpful to know: current challenges, preferred schedule, any recent discharge instructions, and key safety concerns at home.

  2. Professional assessment and care plan - After we gather details, we evaluate needs and build a care plan. A registered nurse can complete an in-home safety/risk assessment, review the plan with you, and tailor it to your goals.

    • We identify risks, potential hazards, and practical ways to improve home safety.

    • We coordinate and communicate with service coordinators as needed to keep the process moving.

  3. Ongoing care and adjustments - We match you with professional direct care workers and a schedule designed around your needs. If health or conditions change, we adjust the plan and staffing accordingly.
