Waiver / Medicaid Recipients

Community Health Choices consists of managed care programs designed for Medical Assistance recipients. Individuals over 65 years old or adults between 21 and 64 with physical disabilities can qualify for long-term services and support. The idea is to serve more people in the communities rather than in facilities. Managed Care Organizations are in charge of the Community Health Choices programs (MCOs). They ensure that Medicaid recipients have better access, support, and coordination of medical care. The goal is to establish a long-term support system where Medicaid recipients can utilize home care agencies, such as Maximum Care, to help increase the quality of life, independence, and health condition.

Long-Term Services and Supports (LTSS) refers to services and support that meet the needs of people with chronic conditions who cannot independently care for themselves. Home and Community-Based Services (HCBS) programs, sometimes called "Waivers,” are one type of LTSS. Waivers provide an alternative to institutional care for individuals who wish to remain at home. Many are unaware of Waivers, how to qualify, or how to access these available programs. Maximum Care can help you navigate through this process and keep you informed about the services presented to you and how to most benefit from them.
